[MS_AccessPros] Looking for a template for documenting my trouble shooting tips

I've been in my current position for a long time; much longer than anyone
else. Because of that I know how to solve most of the technical issues that
come up, that our users encounter, etc. I need to document this in a way so
that others, especially new hires, could reference so they can come up to
speed as soon as possible. I thought I'd look for some MS Access templates
for documenting this, but what I'm finding on the Office website is things
like problem tracking, or issues tracking. That would sort of be good, but
it's more than what I need. At the basic level what I need is something
that will document a symptom that a user encounters, and then lists all of
the possible ways I use to fix that problem. I suppose it should also list
the software system involved.

So I'm wondering is there a MS Access template that does this? We're
currently on MS Office 2010, and are likely to stay here for probably
another year or so.

BTW, I work as a senior programmer analyst, primarily focusing upon ASP.NET,
XAML, C# and SQL Server. Yes, I could roll my own, but in this case I'd
much rather use someone else's if it already exists, as I'm very pressed for
time to work on important projects.
