Bill,
I will try to put it as the background of a report and see if that works. I
will put the report fields on top of the image if I can.
I do not know exactly what you meant by "image control"
I need to print this page out so I am thinking a report would be best.

Could you use the PDF as an image either in an image control or the
background picture of a form? Seems to me that would be the simplest way to
reproduce the layout.
