Yes, use a report if you are printing it out. If you are gathering data or editing it then a form would be the way to go.
An image control is like any other control on a report. You know...text box, label, etc.
That video at data pig is what I was talking about. I should have said capture it as an image file of some kind first. Although a PDF file might have worked with an unbound object frame control. I've never tried it.
