Whenever a database is required you always start with that. Do a thorough job of building an ERM (entity relationship model) which is a diagram of all the tables, their fields and how they all relate. Doing this will save you hours if not days of database design.

I've never built a website that used a database, but there are plenty of tools available for this. In fact, your host probably offers tools for free.

The tools should show you how to build a login page, request and submit pages, etc. A good website forum wouldn't hurt either.
