I installed it and double clicked on the ON and it looked like its turned on but it is not doing anything. Does it have to be in the folder where the DB resides?

Kinda, sorta. There is a logger that you can turn on/off thay will give you a trace. We have the .reg file in our files folder. It's called ShowPlanRegFiles.zip.
Be sure to turn it off when you don't need it as it can be a big drag on performance just like the profiler is in SQLS.
