You can avoid a separate database if you choose to export the merge data to a csv file and then merge the csv with your word template.
Here is a dropbox link to a sample file with the code that accomplishes this. The zip contains a access db and a word template file. Please unzip both in the same directory.
The code for exporting to csv and then merging it is beneath the button "Merge" on the opening form.
The form is crude but the code should give you an idea of how to do this.
