It's not a template. It's a Navigation Form - a new beast in 2010. You can find an option to build a form like this on the Create tab for both Web and Desktop databases.

I just downloaded it the Northwind 2010 Web database and was just looking through it. I like the way it looks, I am wondering what type of the navaigation form templates was used to create the Main form. Any idea?
